Transferring Your Care to Life Balance

We understand that transitioning to a new practice can be a big decision, and we are honored that you’re considering continuing your journey with your current therapist here at Life Balance Counseling.
At Life Balance Counseling, we are committed to offering a structured and supportive environment that fosters growth and healing. We believe in offering compassionate and evidence-based care while maintaining consistent practices and clear boundaries to ensure the best possible outcomes for our clients.
What to Expect at Life Balance:
Sessions/ Office Locations:
We offer both in-person and online sessions. Our offices are located at 823 Filmore Avenue and 2820 West 21st Street in Erie.
Insurance and Payment Information:
Our LSW clinicians are currently eligible to be in-network with Blue Cross/Blue Shield and UPMC commercial plans. Our LPC and LCSW clinicians are eligible to be in-network with these plans as well as with Aetna and United/ Optum commercial plans. We will do our best to verify your insurance benefits, but because insurance plans do not guarantee the quotes they give to providers, we encourage all clients to call their insurance and ask about fees relates to outpatient therapy, including any applicable deductibles.
Unfortunately, our staff are not able to accept Medicare or Medicaid at this time.
Our out-of-pocket rate is $140 per session.
We offer sliding scale/ reduced rates to a limited number of clients based on financial need. These rates are typically $75-115.
To streamline our services, we require a credit card on file for every client. This allows us to automatically charge any fees not covered by insurance, such as copays or deductibles, which will normally be processed within 48 hours of your session or upon receiving remittance from your insurance if the charge is different from what we expected.
Life Balance does not allow clients to carry a balance. This policy helps us maintain a focus on your therapeutic work without the added concern of unpaid bills.
Client Portal Access via Simple Practice:
To make managing your care as convenient as possible, we utilize a secure client portal through Simple Practice. This portal allows you to easily schedule appointments, view your upcoming sessions, and securely communicate with your therapist if necessary.
You will also complete and sign all necessary forms in the portal and can access your billing information here.
Cancellation Policy:
We value your time and the time of our therapists. To respect this, we have a 24-hour cancellation policy. If you need to cancel or reschedule an appointment, we ask that you do so at least 24 hours in advance. Cancellations made with less than 24 hours' notice and no-shows will incur a $75 fee. We do enforce this policy so we like to make sure clients understand it at the beginning of treatment.
Contact Between Sessions:
To ensure that your therapist can fully dedicate their attention to you during scheduled sessions, we generally discourage unscheduled contact outside of appointments except for scheduling purposes. If you require support between sessions, please discuss increasing session frequency with your therapist or ask us about referrals for other services to add additional support.
